Program Description
A program is provided in the home school for students in grades K to 12 with moderate to profound intellectual disabilities, autism or multiple disabilities.
Each student works within the framework of an Individual Education Plan (I.E.P.) where goals are set to meet individual learning needs. Students receive instructional support in the development of communication skills, cognitive/academic skills, fine and gross motor skills, self-care, life skills and socialization skills.
The case manager is responsible for:
- individual teaching to carry out goals and objectives
- coordination of services
- coordination of a comprehensive I.E.P.
- support for teacher, student and parents
- special Education Assistant direction
- advocacy for teacher, student and parents
Service is provided from trained school-based staff, or from an Itinerant Resource Teacher. If an Itinerant Resource Teacher is involved, a school case manager is required. The itinerant service may shift to a consultative role once the I.E.P. is in place.
Referral Process
Students eligible for this service must meet the Ministry definition for moderate to profound intellectual disabilities, autism or physically dependent (multi-handicapped).
