REQUIRED FORMS – 1 FORM PER CHILD PER YEAR

Must be completed at the start of each school year. Click here to fill out the Emergency Release form. Print and return it to your child's school.

1)  Volunteer Form

​This form (mainly for new volunteers at the school), can be submitted via KEV School Cash Online.

It needs to be completed once at each school level (Elementary/Middle/Secondary).  If you have completed a paper or online Volunteer form for this school in previous years, this is still valid and you do not need to complete this form again.

2)  Criminal Record Check

If you have not already done so, you will need to submit a criminal record check (CRC) to your child's school.  This form is valid for 5 years and will be forwarded to your child's next school if appropriate/still valid.  Please see important address detail:
 

If the address on your Driver's License is Port Moody:  You will need to obtain your Criminal Record Check (now called a Police Information Check) online at Police Information Checks - Port Moody Police Department and upload theRequest for Police Information Check - Port Moody.pdf to their website.  On this site, volunteers must click on the orange "Apply Now" button and select "Volunteer in SD43" as the reason for the check.  There is a $10 (plus tax) cost.

If the address on your Driver's License is Anmore, Belcarra, or Coquitlam:  You can obtain the CRC with the Coquitlam RCMP (604-945-1550) in person and no charge with this Criminal Record Check, fee waiver​.
