REQUIRED FORMS – 1 FORM PER CHILD PER YEAR

Must be completed at the start of each school year. Click here to fill ou​t the Emergency Release form.  Print and return it to your child's school.  

Note: you can save this PDF on your home computer and edit it each school year as necessary...a real time saver!

1)  Volunteer Form (if you will also be driving, please use the Volunteer Driver Form)

​This form can be submitted via KEV School Cash Online.  You can also get these forms at the school office.

2)  Police Information Check  (AKA Criminal Record Check)

If you have not already done so, you will need to submit a Police Information Check to your child's school.  This form is valid for 5 years and will be forwarded to your child's next school if still valid.  Please see important address detail below:
 

--If the address on your Driver's License is Port Moody:  You will need to go online at Police Information Checks - Port Moody Police Department and upload the Request for Police Information Check - Port Moody.pdf to their website.  On this site, volunteers must click on the orange "Apply Now" button and select "Volunteer in SD43" as the reason for the check.  Individuals are responsible for any fees charged for this service.  Please consult the Port Moody Police website for a list of fees.

--If the address on your Driver's License is Anmore, Belcarra, Port Coquitlam, or Coquitlam:  You can obtain the check with the Coquitlam RCMP (604-945-1550) in person and at no charge with this Criminal Record Check Fee-Waiver Letter.pdf (take with you to the RCMP)

And if you will be driving students: 

3)  ​Volunteer Driver Certification for Insurance & Vehicle Spec form (if you will be driving students)

This form is a part of the Volunteer Driver Form that is completed online.  You can also get these forms at the school office.

4)  Driver Record (AKA Abstract)

Volunteers who will be driving children other than their own will need to obtain a free Driver's Record at www.ICBC.com​ or by telephoning 1-800-950-1498.  The document can be faxed or emailed to you or to the school (anmore@sd43.bc.ca or fax:  604-937-8001).
